Records, Reports and Certificates
Copies of Postmortem reports can be obtained through the Kankakee County Coroner's Office. Please provide a written statement which includes the decedent’s name, date of death, and your relationship or please fill out the FOIA form, either can be mailed to:
The Office of the Kankakee County Coroner, 3000 S. Justice Way, Kankakee, IL 60901.
Please include your return address along with a phone number where you can be reached. Once the file has been completed, you will be notified of the charges. Please allow 3-5 business days for processing. Release of photos will be at the the discretion of the Kankakee County Coroner, photos can only be released to the legal next of kin. If you are not the legal next of kin, please have the legal next of kin fill out the Release of Privacy Interest Form.
Per the State of Illinois, Public Act 096-1161, fees are as follows:
- Autopsy Reports: $50.00
- Toxicology Reports: $25.00
- Miscellaneous Reports: $25.00
- Photos: $3.00 per photo
- Coroner's Report: $25.00
NOTE: Acceptable methods of payment are: cash, cashier’s check or money order. Please be advised there is a $3.00 transaction fee for all credit cards.
Copies of Death Certificates
Certified copies of death certificates can be obtained by contacting your funeral home or the Kankakee County Clerks Office Vital Records Department.
Funeral Homes and Cremation Centers who are requesting a cremation permit, please send all cremation permit request through IVRS. Fetal death certificates and anatomical gift are still done by paper, and you can fax request to, (815) 802- 7191. Please be advised the cremation fees at this time are $50.00 per cremation, this amount is subject to change. Please mail check with the decedents name and/or contact the Kankakee County Coroners Office to pay over the phone with a credit card. Please remember there is a $3.00 credit card transaction fee.
